Plot

The Pilot introduces us to Tony Soprano, a New Jersey mob boss struggling with anxiety attacks that have become increasingly frequent and disruptive. Following a series of medical tests at the hospital that come up negative, Tony is referred to psychiatrist Dr. Jennifer Melfi. Initially reluctant to open up about his personal life, Tony gradually reveals details to Dr. Melfi as she probes into the causes of his anxiety. Tony’s complex life is riddled with family and business issues. At home, he faces tension with his wife Carmela, who feels neglected by his constant demands and late nights at work. His daughter Meadow is a high-achieving student preparing for college, while Tony's relationship with his mother Livia, who refuses to move into a retirement community, adds another layer of stress. Professionally, Tony struggles with managing the ambitions and decisions made by his nephew Christopher Moltisanti. Uncle Junior’s plan to kill Little Pussy in one of Tony’s friend Artie Bucco’s restaurants also complicates matters. The episode culminates with Tony feeling a sense of relief after speaking with Dr. Melfi, attributing his improved mood to prescribed medication. However, Dr. Melfi explains that the real reason for his better state is the therapeutic effect of their sessions.
